From last month’s delay back to a control

Studios that end in applause have not been tested. The Operational Risk Grid ends by picking one trail and walking it against a delay that already happened. If the map cannot explain the delay, the map is revised in the room, in front of the people who will own it.

This is not a blame ritual. It is a control loop. The first observer named on the trail either did see the failure or did not. If they did not, the owner line is fiction. If they did, and the next physical action was missing, the trail was a slogan.

Teams sometimes ask to use a famous industry accident instead of their own month. We refuse. Famous accidents teach mood. Last month’s hose teaches the grid. Continuity Under Constraint uses the same loop at a shorter length: rehearse a freeze, then correct the owner list before anyone prints a certificate.

Keep the correction. A grid that never fails in class will fail on a Sunday. That is the only pedagogy we trust.
